Frequently Asked Questions

Can I schedule online for any services at any location?

Online scheduling is available for Family Medicine, Pediatrics, and Dermatology across all clinic locations. Other specialties and urgent care appointments cannot currently be scheduled online.

What if my address or phone number has changed?

If your demographic information has changed, do not update it online. Instead, contact the clinic at 801-475-3500 for updates.

What insurance information do I need to schedule an appointment?

We require insurance details for appropriate scheduling. Please provide your insurer's name, policy/member number, and Plan ID/group number.

What if I don't have insurance?

If you are uninsured, select 'Self Pay' when prompted for insurance details and proceed with appointment scheduling. View Self-Pay estimates here.

How do I schedule my physical/annual visit?
  • Adults (18+): Choose complete physical male/female or MCR Wellness for Medicare.
  • Children (17 and under): Select Well Child Check with the appropriate age.
Can my appointment take place over Telemedicine (video chat)?

For visits that do not require physical exams or procedures, telemedicine is an option! Most specialties use Telemedicine; ask the scheduler or your provider if your next visit can take place over video chat.

What if my provider doesn't have appointments for urgent issues?

For urgent or same-day appointments, contact the clinic at 801-475-3500 or schedule an appointment using our Urgent Care check-in. Walk-in access is also available at our Urgent Care clinics.
