Your first visit to Utah Hematology Oncology | Ogden Clinic may come at a time filled with anxiety as you face a tremendous challenge. We would like to make your first and every visit as pleasant as possible.
Education Packet Care TeamSince this can be a very emotional time, we strongly recommend taking a few actions that will help you get the most out of your visit:
- Consider having a family member or friend accompany you who can take careful notes. This is also true for when you receive education from providers and nurses. There is a lot to learn and so much happens in such a short time it is easy to become overwhelmed or confused.
- Please bring your government-issued photo ID; your current insurance card(s)—including pharmacy card, if different; the completed forms; and your initial copay (see your card or contact your insurance company for details).
- If a printer is available, please download and print the Patient Packet here prior to your first visit. This helps us obtain your health information and streamline the patient intake process.
